As a nation, we are shifting the healthcare system beyond hospital to community. To make this shift systematic and sustainable, one needs to be familiar with both the existing efforts of community partners, and the resources in healthcare institutions. This symposium has invited two speakers from the community care setting to share about their efforts in supporting healthy ageing in senior citizens as well as the support they render to palliative patients and their caregivers. Following which, colleagues from SingHealth Community Hospitals will share insights on their efforts on care integration for patients and their model of care transition from hospital to community.
