This talk will focus on why person-centred care (PCC) is important and share the localised definition of PCC unique to SingHealth. It will highlight the two aspects of “Caring through interpersonal relationships” and “Sharing of power, control and care options” to achieve equal partnership with patients. Through stories of patients and caregivers, the talk with discuss the four actionable steps of PCC, as well as how PCC will increase concordance and shared accountability between providers and patients.
