Everybody starts as a child; which is a fundamental branch of population health. Conforming with the Nation’s vision of enhancing population health and reducing the incidences of metabolic syndromes, it is paramount to ensure the capacity and capability of nurses to serve this vulnerable population.
Classically, community care focuses on the prevention and management of chronic diseases. Promoting empowerment in healthy aging and disease control in our adult population. Hence leading our nurses in the community to be experts in the care and management of chronic geriatric issues.
With the vision to augment population health the aim is to provide early identification and intervention for our young to the elders. This would precipitate a holistic model of care for the whole family; from womb to tomb.
Thus, the enhancement of the existing system for care for the mother and child dyad.
