Healthier SG (HSG) was launched in July 2023, with residents aged 60 and above to be invited to enroll with their preferred primary care provider.

To support and in line with the nation’s Healthier SG (HSG) strategy, SingHealth Polyclinics have successfully set up My Care Team (MCT) in all our nine polyclinics to bring our nation together and encourage active living.

Primary care nurses play an imperative role in My Care Team to deliver HSG to our patients to empower them to stay healthy, detect and manage health issues early and chronic condition holistically.
