Social Service Agencies are increasingly seeking to partner with health care institutions to better address community needs especially for clients with complex health and social needs living in the community.

Montfort Care with its network of 3 Family Service Centres, 1 Strengthening Family Centre, 1 Child Protection Services and 8 GoodLife believes strongly in establishing strong community partnership. The presentation shares how Goodlife runs a continuum of services for seniors who are frail and active in partnership with healthcare organisations to better address the seniors’ health and social needs. It draws from the experiences of two sites, Goodlife Makan and GoodLife Studio.
