Advance Care Planning (ACP) is an important process in promoting patient self-determination and facilitating understanding of patients’ values, wishes and care preferences to their families and healthcare team. There are ongoing efforts in bringing ACP upstream into the community. All healthcare providers can play a role in initiating ACP conversations with patients and their loved ones.
