Ms Xinpei is an advance care planning programme coordinator in Singapore General Hospital (SGH). She is a certified ACP trainer and facilitator, and is part of the SGH Renal Supportive Care (RSC) Clinic to provide comprehensive renal care for patients with end stage renal failure. Ms Xinpei is also a member of the SGH ACP Workgroup to direct and chart the implementation of ACP in the hospital.  Apart from direct practice in ACP facilitation and conducting ACP facilitator training for health and social care professionals. She also provides supervision and mentorship for newly trained ACP facilitators across care settings.
