Asst Prof Agnes Koong is a Consultant with SingHealth Polyclinics. She graduated from the National University of Singapore and subsequently completed her postgraduate training in Family Medicine in 2011 and is now a Fellow of the College of Family Physicians Singapore.

She was Clinic Director, Marine Parade Polyclinic between October 2012 to June 2019, and is presently Clinic Director for Tampines North Polyclinic (ready in 2023).

A key driver of programmes supporting patient empowerment and integrated care, she now leads the pilot for Primay Care-based Integrated Care Team, in Marine Parade. This pilot brings together key providers in the community supporting the care of patients with complex care needs.

In addition to her administrative duties, she is also involved in teaching undergraduate and post-graduate students. In 2019, she received the National Medical Research Council (NMRC) research fellowship grant. She is now pursuing a part-time PhD in Clinical Sciences Programme at Duke-NUS Medical School Singapore.
