Dr. Jasmine is a board certified critical care pharmacist and clinician innovator with a focus on AI and digital health innovations. Jasmine is a US-ASEAN J William Fulbright Scholar to UCSF, focusing on Health AI research, governance and research. She has established research collaborations with global experts, published in various high-tier journals and is an active core member of several international committees in AI ethics and regulations. Her research involvement spans across multiple collaborative projects, serving as co-investigator in local multi-centre studies. She currently serves as a research peak co-lead in Singapore General Hospital and an AI EXCO member at SingHealth.

SC 4 - From Medicines to Patients: Building Patient-Centric Monitoring Systems in the AI Era

Healthcare is shifting from episodic, provider-centered care towards continuous, patient-centered care. Advances in artificial intelligence, wearable technologies, digital biomarkers, patient-reported outcomes, and conversational AI now enable healthcare teams to understand how patients experience medications beyond the hospital or clinic. This evolution presents an opportunity to redesign medication management around the patient rather than the prescription.

Drawing on local and global research trends, this presentation explores how patient-centric monitoring can improve medication safety, personalize therapy, and support earlier intervention across the medication journey. Case studies, including AI-assisted medication safety, real-time monitoring of chemotherapy-induced cognitive impairment, and AI-enabled clinical decision support, illustrate how continuous monitoring can transform care while integrating seamlessly into clinical workflows.

The presentation concludes with practical implementation principles and discusses how healthcare organizations can build patient-centric monitoring systems that not only improve outcomes and operational efficiency but also create learning health systems capable of continuously adapting to patients' needs.
