In the transition from acute wards to community care, the practice of keeping patients safe moves from a controlled clinical environment to the unpredictable landscape of the home. While hospitals offer standardized, sterile spaces, the community health nurses must adapt clinical excellence to fit the reality of the “heartlands” – navigating everything from cluttered living spaces to diverse cultural practices and family dynamics. This session explores how our community nurses bridge the gap, turning the community environment into a safe space for recovery through resourcefulness and trust.
