The symposium will bring together healthcare experts that are supporting population and community healthcare from different dimensions. From a wider angle, we will learn to explore the strategic approach to healthcare as seen in the Western Pacific Region and the localised approach of Singapore’s Eastern Regional Healthcare System. Delving in, we will learn how population healthcare strategies segue into personalised healthcare through social prescribing.
Social prescribing is an approach to an individual’s wellbeing through addressing the social determinants of health, the conditions by which individuals are born, live, work and play. Preliminary studies have shown that improving social determinants of health can give rise to better health outcomes. Various modes of implementing social prescribing, including inpatient and outpatient settings will be discussed.
