Ms Zhu Xia

Ms Zhu Xia is an Advanced Practice Nurse in National Cancer Centre Singapore (NCCS.) She works as a full-time Nurse Clinician in the Division of Supportive and Palliative Care for over 10 years. She worked as part of the inpatient palliative team previously to see SGH palliative patients and was seconded to Assisi Hospice for more than one year to support their Home Care service. Currently she is running the DSPC Telemed Nurse-led clinic and following up with all the patients after their initial video consultation.

Presentation Synopsis

Providing Proactive Palliative Care through the Novel Use of Telemedicine for Patients with Advanced Cancer

  • 12 weeks service, helmed by a palliative care APN; supported by a palliative care consultant.
  • Inclusion criteria: All advanced cancer patients who are not known to existing palliative care services can be referred (even those on active treatment)
  • Referral can be made by the medical oncologist directly to the palliative care APN
  • Patient will be seen virtually in telemedicine clinics for their initial consultation
  • Patients / caregivers have an initial video consult via zoom, followed by phone consult after their regular symptom monitoring through patient reported surveys to provide timely adjustments to medications and their treatment care plan and seamless transition to community palliative care services.
